Information collected from you by Dr. Rehana Manji & Associates:

Demographic information such as your name, date of birth, address, phone number, email address

  • We collect this information in order to identify you in our internal systems, provide you with quality eye care that is tailored to your age group and to contact you to confirm your appointments and send appointment reminders. 

  • We may also contact you periodically to inform you of products or services you may benefit from. You can opt out of email or text communications at any time by contacting our office by phone or email.

Health insurance information such as your OHIP number and private insurance plan number

  • This information is collected so that we can direct bill fees for eye care services on your behalf. 

Health information

  • In order to provide a high standard of eye care, it is importance for us to know certain facts about your health including, but not limited to, the type of medications you currently take and your family history 

Financial information such as your debit or credit card details 

  • Due to COVID-19, we are currently only accepting contact-less methods of payment. This will sometimes involve collecting your card details. We will securely store your financial information to be used at future visits only if you give us consent to do so.

Website Traffic Statistics

  • Our website www.rmoptometry.com is hosted by wix.com, which uses essential cookies to track website visits including (but not limited to) the amount of time visitors spend on the website and which pages are viewed. A cookie is a small string of data that is stored on a website visitor’s browser and it is used by the website to recognize returning visitors. These cookies do not collect or store personally-identifying information. If visitors to our website do not wish to have cookies placed on their internet browser, they should adjust their browser preferences to refuse cookies before visiting.

 

Sharing your information with third parties:

Dr. Rehana Manji & Associates will not share any of your personal information with third parties without your verbal or written consent unless we are required to by law or to comply with credit reporting requirements or fraud investigations.

 

There may from time to time be a need for us to share your personal information with other health care professionals in order to provide you with the best possible care. This may be in the form of a referral or consultation report. Your information will be shared in a secure manner and you will be informed before any information is sent.

 

Within the office, your demographic information and information regarding your glasses prescription will be shared with the staff of Pearle Vision Liberty Village in order to provide you with optical services. Please refer to the Pearle Vision Liberty Village privacy policy to learn how they use and share your information.

 

How we protect your privacy:

We will make every reasonable effort to ensure the security of the information we collect from you. This includes encryption of data, using secure methods of electronic communication, securely storing any paper files and shredding any discarded documents that contain your personal information. We will regularly update our security policies and practices as the technology we use evolves.

 

Access to your personal information:

You have the right to view your personal, health and financial information that we have collected. If you wish to view your file, contact us by phone, email or in person and we would be happy to compile that information. It may take us up to 10 business days to prepare this documentation for you.

 

Effective Date:

This policy is effective as of February 1, 2021.  We reserve the right to amend this Policy at any time. Any changes to this Policy will be effective from the time that the revised version of the policy is posted.
